If you are HIV+ and are living in York, Chester or Lancaster Counties of South Carolina, here are the steps to receive services at Catawba Care Coalition:

• Call (803)909-6363 x0 or 1(877)647-6363 x0
• Ask to speak to the Duty Case Manager
• Tell them you would like to make a self-referral
• You will need the following information:
•  Demographic information
•  Contact information
•  HIV verification (CD4, viral load, diagnosis date/place)
•  Insurance information (if any)
• Schedule an intake with a medical case manager
• Bring identification, proof of HIV status, proof of income, and any insurance information to first appointment
• Medical case manager will complete initial paperwork and schedule a clinic appointment for you.
