Support Services are intended to help people living with HIV/AIDS to obtain and maintain medical care. The support services team helps clients reduce barriers to medical care and assist them in accessing the resources they need to live a healthy life with HIV. Case managers coordinate these services and work closely with the clinic staff to ensure high quality care for CCC clients.

Client Advisory Board: This group
meets with the Executive Director every other month. It gives you a chance to share ideas and
opinions for CCC’s programs and services.
Just in its first year, this group helped CCC create a five-year plan,
chose the design for a new billboard, identified new community HIV testing
sites, and planned articles for the quarterly client newsletters. If you want to be part of this group or want

Referral Services: Case managers can help clients with services in the community. For
example, case managers know about drug and alcohol abuse services,
mental health counseling, crisis programs and other special programs
such as Community Long-term Care, social security disability and
Medicaid.

Counseling: Case managers can refer clients to on-site counseling if they are struggling with HIV
issues or life stresses in general. Case managers can also connect
clients to spiritual counseling through the Regional AIDS Interfaith
Network (RAIN) chaplain and RAIN care teams.

Fincancial Assistance: If clients qualify, case managers can help provide financial
assistance. This includes help with housing and utilities on an
emergency basis, medications, other medical and dental care, gas
vouchers for travel to and from appointments, and some food assistance.
A case manager can only offer financial help once a client has provided
proof of income.

New Client Group: CCC hosts a six-session small group with men and women who are newly diagnosed with HIV/AIDS or are new in care with Catawba Care Coalition. Educational sessions include topics related to new HIV diagnosis, HIV medication adherence, disclosure of HIV status, and other psychosocial issues.

Nutritional Needs: Clients are eligible to meet with a registered dietician. Nutritional
supplement drinks, such as Boost, are given out by prescription only.

Transportation: If needed, we can try to help clients with transportation to HIV-related
appointments. Medicaid transportation is used when it is available.
Clients must call their case manager three days in advance to request
transportation.

Support Groups: Support groups offer a way to meet other people living with HIV,
develop support systems and friendships, and learn about different
subjects.

Social Activities: CCC
hosts social events in all three counties throughout the year. Past socials have included a spades tournament,
a talent show and holiday theme parties. All clients, staff and
volunteers are invited to attend socials.

Volunteers: Both community members and clients volunteer to help at CCC.
They provide many services, including transportation, food preparation
for socials, babysitting (for socials and medical appointments), emotional support, community education, and office work.

Personal Pantry: The pantry is a toiletry/cleaning supply closet that is sponsored by
RAIN (Regional AIDS Interfaith Network) and stocked by local churches.
Available supplies usually include toilet paper, paper towels,
detergent, shampoo and deodorant. Clients may receive supplies one time
every other month.

Emergency Food Pantry: Case managers can offer clients a small supply of canned and dry goods in emergency situations.
